You can then schedule an appointment by phone, email, or in person. Please make sure to have the following documents ready:

Private Insurance: you must cover costs upfront, and reclaim reimbursement by insurer.AVBZ: for some care products, AVBZ might be required. AVBZ could be enabled for more than 90 days of treatment.No valid insurance: you must cover costs yourself.
